      The Near-miss Book has been produced by the Health and Safety Executive to help employers and employees record details of workplace near misses. Near misses are less severe than accidents. However, near misses should not be ignored or treated lightly, as they can provide valuable insight into how well you are managing health and safety in your workplace. This record book will allow you to see if there are any patterns in when or how things go wrong. A pattern of near misses provides an early warning that something needs attention. It makes good business sense to be proactive and take action early when problems are likely to be less serious. Near misses may seem trivial but they are a valuable source of information. Taking time to review the underlying causes is likely to reduce risk, improve health and safety, and save you time and money in the long run. Protect your business by encouraging your employees to use this record book and reduce the risk of something going wrong. Please There is no legal requirement to record near misses, however, you do have a legal duty to protect the health and safety of your employees and members of the public who may be affected by work activities. Understanding near misses can help you to do this by reducing the risk of accidents.

Previously published by the department for Work and Pensions (DWP), it has been revised as most existing accident books allow personal details to be seen by anyone reading or making an entry in the book. The Data Protection Act 1998 requires that personal details in accident books must be kept confidential. Individual record sheets can be removed and stored securely. Contains 96 copies of the record sheet, instructions on how to use the book, and guidance on the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 and the Health and Safety (First Aid) Regulations 1991.       The Police and Criminal Evidence Act (PACE) and its Codes of Practice are a vital part of the legislative framework that lays out police powers for combating crime. PACE Code E details the audio recording of interviews with suspects in the police station. This Code of Practice applies to interviews carried out after 00.00 on 2 February 2016. This Code of Practice must be readily available at all police stations for consultation by police officers, police staff, detained persons and members of the public.

      This guidance is mainly for employers, managers and safety representatives, but may also be useful for employees. Employers must comply with the Manual Handling Operations Regulations 1992, as amended by the Health and Safety (Miscellaneous Amendments) Regulations 2002. The guidance explains how to avoid, assess and reduce the risk of injury from manual handling. Changes since the last edition The publication has been restructured into four parts, with the regulations and brief guidance in Part 1 and more detailed guidance to help carry out risk assessments and control risks in Parts 2 - 4. Colour coding helps to identify the different parts. The appendix contains risk filters which help identify those tasks that do not require a detailed assessment. It explains how HSE's assessment tools can be used as part of the risk assessment process. The full risk assessment checklists are now online only and do not appear in the book. It reflects changes introduced for the self-employed by the Health and Safety at Work Act 1974 (General Duties of Self-Employed Persons) (Prescribed Undertakings) Regulations 2015 and the Deregulation Act 2015 (Health and Safety at Work)(General Duties of Self-Employed Persons) (Consequential Amendments) Order 2015.
