The management and implementation of resettlement and reintegration for released prisoners in Europe is increasingly crucial. Effective transition management involves collaboration among criminal justice agencies, statutory and voluntary providers, and other stakeholders in the offender's reintegration. The project “Justice Cooperation Network – European treatment and transition management of high-risk offenders” aims to develop efficient management principles and practices for high-risk offenders leaving custody, which can be shared among project partners across Europe. The project team includes representatives from Estonia, Finland, Ireland, and Mecklenburg-Western Pomerania (Germany), along with associated partners such as the Ministry of Justice and Prison Service of Belgium, the Ministry of Justice and Public Administration of Slovenia, the Ministry of Justice of Slovakia, the Confederation of European Probation (CEP), and Germany's Federal Ministry of Justice and Consumer Protection. The publication is organized according to the final conference program held in Warnemünde in September 2014. It includes introductory addresses, plenary presentations, contributions from four forums, and a summary of reflections from professionals outside the justice system. The final evaluation report by researchers from the University of Greifswald concludes the publication.
